Richard J. Favreau, OD Health Foundation Equipment Donation Criteria

The Richard J. Favreau, OD Health Foundation is dedicated to matching donations of optometry
lane equipment with healthcare sites in need that are, or will be, partnering with our Foundation.
If you are interested in donating optometry lane equipment, we kindly ask that you complete the
donation form with the required details to determine if there is a match. We review submissions
on a rolling basis and will respond in a timely manner.

This information allows us to assess whether the equipment can be integrated into our
programs and helps us track donations for acknowledgment and reporting purposes.

Equipment Donation Criteria:

1. Condition: Equipment must be in good working order, clean, and free of damage. We
only match items that are ready for immediate use.

2. No Storage: Due to limited storage capacity, we can only match items that directly meet
our current needs. Donations will be matched with healthcare sites as soon as possible,
and an equipment transportation plan will be made with the site.

3. Quantity: Donations should be in quantities that are manageable and can be distributed
or utilized promptly. Large donations will be assessed on a case-by-case basis.

4. Pre-Approval: All donations must receive prior approval from our Foundation’s Board to
ensure they align with our needs. Please contact us for approval before delivering any
items.
