The Richard J Favreau, OD Health Foundation Children’s Vision Assistance Program Grants aim to provide supplemental funding for vision and medical eye care for children under 18 in New Jersey. By ensuring that children receive necessary vision and eye care, we can continue to promote equal opportunities for success in school and life, ultimately contributing to healthier, more informed communities.

Grant Criteria:
1. Application shall be completed by a licensed New Jersey optometrist for services already performed for the child patient.

2. One patient per application.

3. Applications are accepted on a rolling basis and will be reviewed in a timely manner (approximately 7-10 days).

4. Receipts and documentation of services are required and must be provided as an upload with your application. Providers must not submit Protected Health Information (PHI) to the foundation application reviewer(s). Please have these documents ready for upload before starting the application.

5. The services receipts and documentation will be used to determine eligibility and potential grant amount.

Grant Disclaimers:

Annual services grant distribution will be based on viable applicants, number of applicant submissions, and the health of the foundation’s funds.
Grant amount per patient will be made on a case by case basis, and will follow the below fee schedule.
This is a needs based grant and should not supplement a third-party entity.
One patient per application.
Submission of a grant application does not guarentee grant approval.
Applying or having received grant funds in the past does not guarantee future funding.
Funding criteria and availability per year may vary.
The approved grant check will be written out to the provider, after receipt of a completed Form W9.
If grant funding does not fully cover the requested amount, the provider must absorb the remaining costs and not bill the patient, consistent with the commitment to charitable care.

Grant Fee Schedule
The grant is for lab invoices and minimal or residual chair costs.
Comprehensive eye examination (Up to $75)
Contact lens exam, fitting and materials (Up to $300)
Medical procedures and imaging (Up to $150)
Frames (Up to $150)
Eyeglass lenses (Up to $150)
Other – to be specified (Up to $150)
