Richard J Favreau, OD Health Foundation Children’s Vision Assistance Program
The Richard J Favreau, OD Health Foundation Children’s Vision Assistance Program Grants aim to provide supplemental funding for vision and medical eye care for children under 18 in New Jersey. By ensuring that children receive necessary vision and eye care, we can continue to promote equal opportunities for success in school and life, ultimately contributing to healthier, more informed communities.
Grant Criteria:
1. Application shall be completed by a licensed New Jersey optometrist for services
already performed for the child patient.
2. Applications are accepted on a rolling basis and will be reviewed in a timely manner.
3. Receipts and documentation of services are required and must be provided as an
upload with your application. Providers must not submit Protected Health Information
(PHI) to the foundation application reviewer(s). Please have these documents ready for upload before starting the application.
4. The services receipts and documentation will be used to determine eligibility and
potential grant amount.
Grant Disclaimers:
Annual services grant distribution will be based on viable applicants, number of applicant submissions, and the health of the foundation’s funds.
Each applicant is limited to one application per year. Applying or having received grant funds in the past does not guarantee future funding. Funding availability per year may vary.
The approved grant check will be written out to the provider.
